Insurance

Dr. Perman is an out-of-network provider with all insurance companies, and he is opted out of Medicare. Dr. Perman's patients may submit their invoices to their insurance company to receive reimbursement for treatment provided.

Payment Options

Dr. Perman accepts Zelle, PayPal, and check. Patients who want to pay online with a credit card can request a PayPal invoice at a slightly higher rate. Dr. Perman's fee is $365.00 for the initial 1-hour and subsequent 45-minute online appointments.
